
About the Role
Reporting to the Practice Manager & Medical Director, you will shape the organisation’s hospital interface strategy while driving sustainable referral growth across acute and subacute settings in this pivotal leadership role at the intersection of commercial growth, hospital partnerships, and complex care pathways.
Key Responsibilities:
Lead complex hospital discharge and transition pathways for high needs participants.
Build, nurture, and expand partnerships with hospitals, discharge teams, allied health, and NDIS stakeholders.
Identify and develop referral opportunities across acute and subacute settings.
Collaborate with clinical and operations teams to ensure seamless onboarding and continuity of care.
Act as the organisation’s subject matter expert on NDIS regulation, funding pathways, and compliance.
Support marketing and brand activity, including maintaining a professional online presence.
Requirements
Essential:
Strong understanding of NDIS processes, funding pathways, and compliance requirements.
Demonstrated experience in hospital liaison, partnerships, or discharge coordination within health, disability or community services.
Exceptional business development, stakeholder engagement and communication skills.
Access to a reliable vehicle and a valid driver's license.
Valid Police Check & NDIS Worker Screening Check.
Desirable:
Existing networks within public hospital or healthcare settings.
Clinical or allied health background.
Tertiary qualifications in Health, Community Services or related field.
